$200 Behavioral health evaluation - Comprehensive assessment to help the applicant give context and substance to their past mental health encounters. The applicant can potentially use these evaluations in their quest to obtain waivers as appropriate.
Currently, these are only offered on a Cash-Pay basis through HIPAA compliant tele-health service.
The Applicant will receive a comprehensive Intake with Consent, ROI, and CC Auth Form. These must be completed prior to the appointment per federal law.
The order of operations for this process is as follows:
Applicant initiates contact by selecting "Request a Consult for Iowa National Guard Applicants" (the via button at the top of the page)
Requests are reviewed for appropriateness of service by Raven Psychiatry and assigned an evaluator (board-certified psychiatric nurse practitioner).
Upon approval of Raven Psychiatry, the following documents are sent to the applicant:
Intake Form with Consent for Services
Release of Information (ROI)
Credit Card Authorization Form
These documents must be fully completed and electronically signed prior to scheduling the evaluation in accordance with federal compliance requirements.
Upon receipt of completed paperwork, the applicant will receive a scheduling link via email. The applicant will be able to self-schedule in the online portal. Payment is required upon booking and ensures an appointment slot.
The appointment link is part of the HIPAA-compliant platform. It is delivered to the applicant’s email upon successful booking. The applicant is responsible for ensuring their device can run the platform reliably.
Applicant logs into the virtual waiting room 5 minutes prior to scheduled time.
Evaluation is conducted with full clinical rigor and objectivity over 45–60 minutes.
Upon completion, the evaluator will send the signed and finalized clinical note to:
The applicant, and/or
The recruiter, and/or
Any party explicitly named in the ROI form
By adhering to the outlined steps and maintaining transparency, applicants can obtain a document of value that may aid in the waiver process where appropriate, always in service of national readiness and individual honor.
